It's one of the most common things I hear from patients. "I started feeling better after two days, so I stopped the antibiotics. Why finish the whole course?" It's a reasonable question — and the answer matters more than most people realise.
Why you feel better before the infection is gone
When you take an antibiotic, it starts killing the bacteria causing your infection almost immediately. Your symptoms — fever, pain, fatigue — begin to improve as the bacterial load drops. But "feeling better" doesn't mean all the bacteria are dead. It means enough of them have been killed that your immune system has some breathing room.
The bacteria that survive the first few days of an antibiotic course are often the tougher ones — the ones that are slightly harder to kill. If you stop your course early, you leave these survivors alive. They can multiply, and because they've been exposed to the antibiotic without being fully eliminated, they can develop resistance to it.
What is antibiotic resistance and why should you care?
Antibiotic resistance is one of the most serious global health threats today. It happens when bacteria change in a way that makes antibiotics less effective against them — or completely ineffective. The World Health Organisation has called it a crisis that threatens decades of medical progress.
In India, this problem is particularly acute. Antibiotics are among the most commonly sold medicines, and a significant number are dispensed without a prescription. But isn't taking antibiotics for longer also harmful?

Antibiotics don't just kill harmful bacteria — they also affect the good bacteria in your gut, which is why some people experience stomach upset or loose stools during a course. There is a reasonable ongoing debate in medical research about whether shorter antibiotic courses can be just as effective for certain infections.
However — and this is important — that decision should be made by your doctor, not by stopping when you feel better. If your doctor has prescribed a 7-day course, complete 7 days. If you are concerned about side effects or the length of the course, speak to your doctor. Do not make that decision unilaterally.
Practical tips for completing your course
- Set a reminder — take your antibiotic at the same time every day so it becomes part of your routine.
- Note the end date — write on the strip when you started and when you should finish.
- Don't save leftover antibiotics — never save incomplete antibiotic courses for "next time." Infections are caused by different bacteria, and using the wrong antibiotic — or an incomplete dose — is worse than not taking one at all.
- Take with or without food as directed — some antibiotics must be taken with food, others on an empty stomach. Check with your pharmacist if you're unsure.
What if I miss a dose?
If you miss a dose, take it as soon as you remember — unless it's almost time for your next dose. In that case, skip the missed dose and continue your regular schedule. Never take two doses at once to make up for a missed one.

Feeling better is great. It means the medicine is working. But it's not the finish line — it's the halfway point.
